• The applicant's current Passport

• Two passport sized colour photographs - measure 45 millimetres (mm) high by 35mm wide (the standard size used in photo booths in the UK)
• Must be printed to a professional standard
• Must be taken within the last month
• Must be in colour on plain white photographic paper with no border
• Taken against a plain cream or light grey background
• Identical
• Clear and in focus
• Without any tears or creases
• Unmarked on both sides (unless a photo needs to be countersigned)
• Unaltered by computer software

Note: The photographs must adhere to the Passport Office guidelines. Refer to the Passport photograph Guidelines for clear instructions.

• Permitting D&V to submit the application and collect on your behalf.
• Complete and sign the Letter of Authorisation provided.

One completed and signed Original Passport Application form.

• Application form can be obtained from your local Post Office/British Consulate.
• Must be completed in black ink.
• Correction fluid not permitted. Must be completed in block capital letters.
• Must contain all answers within the white boxes on the form.
• Provide UK daytime contact phone numbers where requested.
• This must be signed by the same parent that signed the application form.

The Passport Office operates an appointment system. The processing time quoted for the Fast Track & Premium Services are dependent on the next available appointment and cannot be guaranteed – current waiting time is up to 2 weeks.

• Complete D&V online order; by filling your personal details; choose your delivery options and make your payment.

Receive Confirmation:

• You will then receive order confirmation via email along with visa application package please print the online order confirmation, fill visa application form print, date and sign.

Send your documents:

• Mail us your original documents using reliable courier(s) such as Royal Mail Special Delivery Guaranteed - Next Day Delivery.
• By submitting this form you accept the D&V Terms and Conditions- Privacy Policy and declare that the information provided is correct. You grant authorisation to D&V to submit visa application(s) to embassies/consulate(s) of selected countries on your behalf.
